Database Update for Emergency Dispatch and Response: Residential Addresses

Residential Emergency Contact Info

Though it is not required that you provide any of the information requested below, we would recommend providing as much as you are comfortable sharing. Rest assured that this data is only accessible by Police, Fire, EMS, and dispatchers and is only used to better protect and serve in case of an emergency at your home or business.

Even if you choose to only supply an emergency phone number, you must scroll to the bottom and press submit. If you have any questions or concerns, please call dispatch at 817-274-2511 or stop by our offices at 2600 Miller Lane during normal business hours.

If you prefer to fill out a printed form, please click here. If you need to submit information for a commercial or business address (rather than residential), please click here.

Include apartment or suite numbers.
Provide name and year of birth. If anyone has mobility issues or other special needs, let us know.

CodeRED is a reverse 9-1-1 system that helps us keep you informed during emergencies or hazardous situations. Sometimes it’s used for general information (e.g., mosquito spraying or event reminders). However, you can choose to receive ONLY emergency updates. If you want to be able to manage your alerts via this system (including subscribing to alerts from other nearby cities), please set up an account here.
Emails will only be used as a last resort if nobody can be reached or to follow up after the incident.
If you have a monitored alarm, let us know how to reach them.
Would ONLY be contacted if residents / occupants listed above cannot be reached.
Let us know anything that might assist first responders. Examples: - Do you have large or aggressive dogs, venomous snakes, or other potentially dangerous pets. - Are there non-residents who have reason to be on-site (e.g., house or pet sitters or cleaning/lawn services? - If you have solar panels installed, where is the control panel? - If you ever store large quantities of ammunition, combustibles, or explosives, where are they located?
